nysmallbusinesshealth.com (“the site,” “we,” “us”) is an independent information resource for New Jersey employers, published by {{PUBLISHER_ENTITY}}. We are not an insurance agency, and we do not sell, solicit, or negotiate insurance.
This policy explains what information the site collects, how it is used, who receives it, and the choices you have.
The short version
- If you submit our form, we collect business contact details and a few facts about your company’s coverage, and we share them with a licensed New Jersey insurance producer so they can follow up with you.
- We don’t ask for health information, and we ask you not to send any.
- We use a small number of service providers to run the site and handle inquiries.

Information we collect
Information you give us
When you submit the form on this site, we collect:
- Your first and last name
- Your work email address and phone number
- Your business name and ZIP code
- Your role at the business
- The number of employees on payroll, your current coverage, and your renewal month
- Whether you’re looking for coverage for employees or for yourself
- A record that you agreed to be contacted, including the date and the version of the consent language you saw
If you email us, we receive whatever you include in your message.
Information we don’t want
We don’t ask for health information about you or your employees, and the site isn’t designed to receive it. Please don’t include medical conditions, claims history, or other health details in anything you send us. If we receive health information, we won’t use it to route your inquiry, and we’ll delete it where we can.
Information collected automatically
When you visit the site, our hosting provider and security tools receive standard technical information, such as your IP address, browser type, device type, the pages you visit, and the time of your visit. Our form uses Cloudflare Turnstile to tell people apart from automated spam, which involves Cloudflare processing information about your browser and device.
How we use information
We use the information we collect to:
- Send your inquiry to a licensed New Jersey insurance producer who can follow up with you
- Send you a confirmation that we received your inquiry, or let you know if your request isn’t one the producers we work with handle
- Keep a record of your consent to be contacted
- Protect the site and our form from spam, abuse, and security threats
- Understand, in general terms, how the site is used, so we can improve it
- Comply with legal obligations and enforce our terms
We don’t use your information for advertising on other websites, and we don’t build profiles about you.
How we share information
With a licensed insurance producer
The main purpose of our form is to connect your business with a licensed New Jersey insurance producer. When you submit it, your inquiry is shared with that producer so they can contact you about group health coverage. Once the producer receives your information, their own privacy practices also apply to how they use it.
With service providers
We use service providers to operate the site and handle inquiries, and they receive information only as needed to do that work. They include:
- HighLevel, which stores inquiries and sends our emails
- Our website hosting provider
- Cloudflare, which provides spam and bot protection for our form
- Email delivery services, which carry messages to your inbox
For legal reasons and business changes
We may disclose information if required by law, subpoena, or legal process, or if we believe in good faith that disclosure is needed to protect the rights, property, or safety of the site, its users, or others.
If the site or the business that publishes it is sold, transferred, or reorganized, the information we hold may be transferred as part of that transaction, subject to this policy.
Your choices
- Emails: we don’t send marketing newsletters. The emails you receive relate to the inquiry you submitted, and they include an unsubscribe link.
- Being contacted: if you no longer want to hear from us or the producer about your inquiry, email {{PRIVACY_EMAIL}} and we’ll stop, and tell the producer that you’ve asked not to be contacted.
- Not submitting: you can read everything on this site without giving us any personal information.
Cookies and similar technologies
The site uses a small number of cookies and similar technologies that it needs to work, including the spam protection on our form. We don’t use advertising cookies or third-party advertising trackers.
Most browsers let you block or delete cookies. If you block the ones our form relies on, the form may not work.
Some browsers offer a “Do Not Track” setting. Because the site doesn’t track visitors across other websites, that setting doesn’t change how the site operates.
How long we keep information
We keep inquiry information for 5 years after our last contact with you, unless you ask us to delete it sooner or we need to keep it longer to meet a legal obligation, resolve a dispute, or enforce an agreement. Consent records may be kept longer to show that we had permission to contact you. Technical logs are kept only as long as our hosting and security providers retain them.
How we protect information
We use reasonable administrative, technical, and physical safeguards to protect the information we hold, including encrypted connections to the site and limited access to inquiry records. No method of transmission or storage is completely secure, so we can’t guarantee absolute security.
Your rights
You can ask us to:
- Confirm whether we hold personal information about you, and give you a copy
- Correct information that’s inaccurate
- Delete information we hold about you
- Stop contacting you
Send requests to support@nysmallbusinesshealth.com. We’ll confirm your request, may ask for information to verify your identity, and aim to respond within 45 days. Deleting information from our records doesn’t delete copies a producer has already received; if you’d like that, tell us, and we’ll pass your request along, or you can contact the producer directly.
Children
This site is intended for businesses and adults. It isn’t directed to anyone under 18, and we don’t knowingly collect personal information from children. If you believe a child has sent us information, contact us and we’ll delete it.

The site links to other websites, including government sources and Get Covered New Jersey. We aren’t responsible for their privacy practices, and we encourage you to read their policies.
Changes to this policy
We may update this policy from time to time. When we do, we’ll change the effective date at the top of this page. If we make a significant change to how we use or share personal information, we’ll post a clear notice on the site.
